The single < 4.0 ng/mL cutoff is the historical threshold and is no longer used on its own. Age-specific reference ranges (rising with each decade), PSA density, free-to-total PSA ratio and PSA velocity all refine interpretation, and current guidance treats screening as a shared decision rather than a single number. Read a PSA against the age-specific range your laboratory reports.
